Please complete this form (you do not have to nominate someone for every office) and return it to info@sfpa.com or via USPS (ELECTRONIC SUBMISSIONS ARE PREFERRED) to the address below no later than October 7, 2016.  If sending via email, please type Elections in the subject line. Nominations will also be taken from the floor at the Annual Meeting on October 14, 2016, after which nominations will be closed.
